3 copies of Statement of Assets, Liabilities and Net Worth (SALN)(form is downloadable at csc.gov.ph)
Transcript of Records (Certified True Copy from the School Registrar) Diploma (Certified True Copy from the School Registrar) 2 Photocopies of PRC License and Board Rating 1 Photocopy of Pag-ibig, Philhealth and BIR Form 2316 (from previous or current employer) Service Record and Clearance (if previously or currently a government employee) Birth Certificate (PSA copy) and Authenticated Marriage Certificate NBI Clearance Result of Physical Examination (Subject to Favorable Result of Physical and Mental Examination)Please be informed that the Request for Physical and Mental Examination is available at the HRD Office.
